| 2. | Proof of U.S. Citizenship | A) Certified copy of Birth Certificate (abstracts are not acceptable)
B) Prior Passport
C) Naturalization Certificate (Only Certified copies showing an agency seal are acceptable).
NOT ACCEPTABLE: Hospital Certificates, Baptismal Certificates, Notification of Birth Certificates, or photocopies of Certified Certificates.
Documents proving U.S. citizenship will be returned with the issued passport. |
| 3. | Two Identical Passport Photos | Photos must be 2" x 2" in size, clear and in color with a white or off-white background and represent a good likeness of the applicant. Regular candid photos and do-it-yourself booth photos are NOT ACCEPTABLE. Passport photos may be taken at City Clerk's Office at a cost of $13 or click here for a list of recommended photo services. |
